BY SUBMITTING THIS FORM, YOU AND YOUR TEAM AGREE TO THE FOLLOWING TERMS AND CONDITIONS, INCLUDING THE ASSUMPTION OF RISK AND RELEASE OF LIABILITY:
1. Player Eligibility: All players in the 50+ (masters) division must be age 50 or older by February 20, 2026. All players in the Open divisions (A divison, B division, Women's division) must be age 21 or older by February 20, 2026. Players must present valid photo identification for proof of age upon request at check-in. Any team found to have ineligible players will forfeit their place in the tournament without a refund.
2. Division Placement: The Adirondack Ice Bowl, LLC reserves the right to re-evaluate and move teams to a different division based on overall player skill level as deemed necessary by tournament organizers to ensure fair and competitive play for all participants.
3. Assumption of Risk and Release of Liability: I understand and acknowledge that participation in the Adirondack Ice Bowl hockey tournament is a strenuous and hazardous recreational activity. I am aware of the risks involved, which include but are not limited to:

Physical exertion and potential for overexertion.

Falls, collisions, and other injuries from contact with players, the ice, or equipment.

Exposure to extreme weather conditions, including cold temperatures, wind, and sun.

Property damage or loss.
I voluntarily participate in this event with full knowledge of the risks involved and hereby agree to assume any and all such risks of injury, disability, death, and/or property damage.
In consideration for being allowed to participate, and to the fullest extent permitted by New York State law, I, on behalf of myself, my heirs, next of kin, assigns, and personal representatives, hereby release, waive, and forever discharge Adirondack Ice Bowl, LLC, its subsidiaries, all 2026 Adirondack Ice Bowl Sponsors, the Village of Eagle Bay, the Town of Inlet, and all of their respective members, officers, directors, employees, volunteers, agents, or any other representatives (the "Released Parties"), from any and all claims, demands, losses, or expenses arising from my participation in this tournament, including those arising from the ordinary negligence of the Released Parties.
4. Weather and Cancellation Policy: The Adirondack Ice Bowl is a pond hockey tournament and is entirely dependent on the natural elements. While every effort will be made to hold the event as scheduled, we cannot control the weather. In the event of tournament cancellation due to unsafe weather conditions or other unforeseen circumstances, teams will have two options: a full refund of their registration fee or a credit to roll their registration over to the following year's tournament.
5. Team Withdrawal Policy: Teams wishing to withdraw from the tournament must notify the organizers in writing. Our refund policy is as follows:

Greater than 3 months before the tournament (before November 20, 2025): A full refund will be issued.

Between 3 months and 45 days before the tournament (between November 20, 2025, and January 6, 2026): A 50% refund will be issued.

Less than 45 days before the tournament (after January 6, 2026): No refund will be issued.
6. Publicity and Image Rights: The names and likenesses of my team and its members can be used for tournament promotion via the official website, public relations, and media campaigns without further compensation.
7. Medical Consent: I agree that in the event of injury, medical personnel may provide necessary medical treatment.
8. Acknowledgment and Consent: I have read, understand, and agree to the terms of this waiver, release, covenant not to sue, and indemnity agreement, and I sign it freely and voluntarily. I am at least 21 years of age.
